Video Disclaimer and Permission

The following form is used as a disclaimer/permission/release for your video recording from your private session with Judith Aston. Due to the sensitive nature of these private sessions, we as that you complete the form below.

Additionally, we want to make sure that you understand our business will not distributed this video to anyone. You will be the sole person to receive this video reminder with this signed electronic release below. We hold your privacy in high regard. Please review the following form, and sign with your name at the bottom. Once you press the submit button, we will receive your accepted form and we will send you a link at which time your link will expire within 7 days. If you were not able to watch the video in that time frame, please write to us at office@astonkinetics.com.
